Surgery on the nose can be purely cosmetic, purely functional, to improve breathing, or a combination of improving both function and appearance. Improving the function of the nasal airway frequently involves surgically straightening the septum (septoplasty) when significant external deformities of the upper third of the nose are present then osteotomies or surgically re-breaking the nose is often required in effort to achieve both a straighter septum and aesthetically straightened appearance of the nose on the outside.

Bilateral Complete Cleft
Clefts can be through the lip alone, through the palate alone or a combination of lip and palate. Clefts of the lip are classified as incomplete when a bridge of skin exists beneath the nostril whereas complete cleft lips go all the way through the sill or floor of the nose and result in greater nasal asymmetry and deformity. Clefts can exist on one or both sides of the lip. Bilateral exists on both sides of the lip. Complete clefts that go through both lip and palate also cross the alveolus or gum.

Cleft Septorhinoplasty
Definitive cleft septorhinoplasty is often needed at the time of skeletal maturity to both improve the nasal airway and symmetry of the nose. The septum is frequently deviated severely in cleft noses which can lead to nasal obstruction and the nostril on the cleft side is often flattened, underprojected and asymmetric.
