Medical Records Request

Request a copy of your medical records from Texas Tech Physicians Clinics. Learn more about the process for each clinic location.

 

Amarillo Medical Records Request

The Amarillo campus follows the process below for requesting copies of Medical Records:

  1. The requester must complete a HIPAA Authorization Form and provide a valid photo ID for identity verification. 
  2. Once received, the Medical Records team places the request in the processing queue. The team has 15 business days from the date of receipt to complete the request.

Incomplete HIPAA Authorization forms or missing IDs will delay processing.

A completed ROI may be submitted:

Permian Basin (Midland/Odessa) Medical Records Request

Permian Basin (Midland/Odessa) Process for Requesting  copies of Medical Records

  1. The patient signs the HIPAA Authorization Form.
    1. Forms may be obtained:
  2. The patient completes the personal information section.
  3. The patient selects whether records are being sent or received.
  4. The patient provides the requesting clinic’s full information (name, address, phone, fax).
  5. The patient selects the information to be released (e.g., all records, progress notes, labs, immunizations).
  6. The patient signs, prints their name, and dates the HIPAA Authorization Form.
  7. If a patient is requesting their own records, they must present a driver’s license or ID that matches the signature on the form.
  8. A TTUHSC employee verifies with the patient which specific clinic(s) they need records from (e.g., Family Medicine, OB/GYN, Psychiatry).

Once complete, staff carefully copy, safeguard, and release the information to the authorized recipient.

Lubbock Medical Records Request

Lubbock patients can request medical records online, in-person, or via mail.

Texas Tech Physicians – Lubbock partners with MRO to provide an efficient and secure online process.

IN PERSON / VIA MAIL Medical Record Requests

If a patient is unable to complete an online request, they may:

  • Visit the Release of Information Office (ROI) to complete an authorization in person, or
  • Mail or fax a completed authorization directly to the ROI Office.

Written Authorization Requirements:

  • All requests must be submitted in writing or through the online MRO system.
  • Authorizations must include all legally required elements.
  • Forms must be dated and signed by the patient or the patient’s legally authorized representative.
  • For minors, a parent or legal guardian must sign.

Patients may contact the Release of Information Office at 806.743.2608 to have the authorization form emailed or faxed to them.
