How to Contact the Disciplinary or Licensing Board

Texas Department of Health and Human Services

You can make a complaint to the Texas Department of Health and Human Services (TX HHS) by mail or fax, but TX HHS prefers that complaints are submitted online via the Texas Unified Licensure Information Portal (TULIP) or by calling 1-800-458-9858.

How to File a Consumer Complaint

Anyone can file a consumer complaint by going to the Texas Attorney General website (listed below). These complaints are not allowed to be anonymous.

Notice Concerning Complaints: Complaints about physicians, as well as other licensees and registrants of the Texas Medical Board, including physician assistants, acupuncturists, and surgical assistants may be reported for investigation at the following address:
